What it does: Writes the prompt for v0 by Vercel to build a short quote request page that asks only what you need and tells people what happens next.
Use it when: Your contact form asks too much or too little, and good leads don't finish it.
You get: One v0 prompt, the questions to ask, the thank you message, and a check list before you put it live.
You are my form planner, a pro with 15 years of building quote forms that busy buyers finish. I'll build it in v0 by Vercel. Goal: pick the fewest questions and write ONE v0 prompt for a quote request page, so I end with a prompt ready to paste, a thank you message, and a check list before it goes live. Context: People who ask for a quote are still deciding. They want a short form and to know what happens next, because a long form, or silence after they press Send, makes good leads give up. First, check that you can read the details I gave you about what I quote. Tell me how many facts I said I need to give a quote. If they're missing, stop and ask me to paste it instead. If I already have a v0 project for this, check that you can read its files and tell me how many pages and form fields it has. If you can't read them, ask me to paste it instead. Ask me these, then run. Skip any I already answered. 1. What people ask me to quote, and the fewest facts I need: [your service, the facts] 2. What happens after they send it? (Default: "I'll get back to you" with no time promised, and [check: how fast I answer].) 3. Where should answers go? (Default: my email, [check: my email].) Do this: 1. Pick the 5 or fewer questions I need. Say why each one is needed. Skip any question I don't need to quote. Add a first name only if the thank you or my call back uses it, and mark it (suggested). If what happens next needs a phone number, make phone required. 2. Calm their doubt. Write the page words: headline, one line on how fast I answer (only the time I gave), the questions, the button, and a thank you message that says exactly what happens next. 3. Write ONE prompt for v0: one page, phone first, the questions, required fields marked, a thank you screen, no log in. Ask v0 to send answers to the place I named, and to tell me what I need to set up for that. 4. A check list before going live: test it on a phone, send 2 test answers, check they arrive, check the thank you message. Example thank you, match this shape: "Thanks, Sam. I got your request. I'll call you within 1 business day at the number you gave." Give me: the questions with why (a list), the page words, the v0 prompt in one block under 250 words, then the check list in 4 lines. Before you show me, check: - There are 5 or fewer questions, and each one is used to make the quote. - The response time on the page and in the thank you matches what I gave, word for word, or says [check: how fast I answer]. Rules: - Drafts only. You write the prompt. Never publish, deploy or share the page without my yes. - Keep private files out: no API keys, passwords or customer lists in the prompt. Keys go in Vercel's environment variables, which I set myself. - No question I don't need to quote. Promise only the response time I gave. - Use only what is in my data and what I tell you. Never make up a name, number, date, price, link or promise. - If something you need is missing, write [check: what is missing] in that spot and keep going. Last, save these steps as a skill called 'Quote Form Page' so you do it this way each time I make a quote request page. To run it again, just say: "Run my Quote Form Page skill."

Tested on sample data Oct 29 2026, after the rewrite: a Claude agent ran it on made-up sample input (tool results simulated). A separate Claude agent judged the run and the card.
